A Support Coordinator is a NDIS-funded professional who helps you understand and implement your NDIS plan. Think of them as your personal guide to the disability sector. Their goal is to build your confidence and independence so you can lead the life you want. They work collaboratively with you to identify the best service providers in your community and ensure all your supports are working together to meet your goals.

Support Coordinator Services

Support Coordination is a specific category of funding within the NDIS. Unlike many other health services, it is only available to participants who have a specific "Support Coordination" budget included in their approved plan.

NDIS (Registered & Non-Registered): We help you understand and implement your plan. We support Agency, Plan, and Self-managed participants under the "Capacity Building" budget.
Specialist Support Coordination: For participants with complex or high-risk situations, we provide Level 3 Specialist support. This is delivered by qualified professionals to manage barriers and reduce risks.
Aged Care (Care Management): We assist seniors with complex needs to coordinate their Home Care Package (HCP) services. We ensure your supports are integrated and your goals are being met.
WorkCover & CTP: We can assist in coordinating multi-disciplinary care for complex recovery cases following a workplace or road accident. This requires specific approval from your insurer.
Mobile Visit: We meet you where you are most comfortable—at home, in the community, or at your workplace. This allows us to better understand your environment and tailor your support.
